Case Management

Case Management is a service that assists eligible clients to gain access to needed medical, social, educational, and other services. This is a wrap around service offered in addition to traditional therapy.

Our case managers are experienced and knowledgeable about community resources.  Their goal is to CALM: Coordinate, Assess, Link, Monitor.  This is a valuable resource for clients with a variety of needs who are not familiar with the resources around them.
